Qualifications
- Graduate of an accredited school of nursing
- Minimum of one (1) year recent experience in a home care setting within the period of two (2) years

Responsibilities
- Assisting in assessing, planning, implementing and evaluating patient care
- Conducting and documenting ongoing data collection related to patient/family reporting to the RN
- Providing care to age-specific patient populations
