Qualifications
- LVN License with current board certification in the state of California
- Current American Heart Association BLS
- Up-to-date TB test and immunizations
Responsibilities
- Facilitating hospital discharge process and patient assessment/engagement to program
- Performing medical assessment intake and discharge plan of care
- Maintaining current guest chart via existing CRM database
